Requirements:
1. Duly accomplished Application Form.
2. Two (2) recent 1” X 1” ID picture
3. Report Card (FORM 138) at least 2nd Grading Period.
4. Endorsement letter from the Valenzuela City School Principal/Registrar that the applicant belongs to the graduating class and that he/she obtained the required General Weighted Average (GWA) of 85% and above.
5. Certificate of Residency and Indigency from the Barangay Chairman.
6. Latest Income Tax Return (ITR) or Certification on Non-filing of ITR both parents (Certified True Copy).
7. Location map of residence.
8. Authenticated NSO Birth Certificate.
9. Certificate of Good Moral Character from the School Principal or Guidance Counselor.
10. Medical Certificate from Government Hospital or Clinic.
11. School ID of Applicant and Voter’s ID of parents.
